transport, wedstrijdstress of stalverandering,
Medicatie of voeding
Daarnaast blijft management cruciaal:
What does a raised ACTH value really mean?
More and more horses are now being tested for Cushing’s, or in medical terms PPID (Pituitary Pars Intermedia Dysfunction). This hormonal condition mainly occurs in older horses and ponies and can lead to muscle loss, coat changes, listlessness and even laminitis.
Yet the diagnosis sometimes causes confusion:
“My horse’s ACTH value is too high — does it definitely have Cushing’s then?” Not always. A raised value is an important signal, but no certainty that your horse has PPID.
🔬 What is ACTH?
ACTH stands for AdrenoCorticoTropic Hormone. It is a substance produced by the pituitary gland (a small gland in the brain) to prompt the adrenal glands to produce cortisol, the natural stress hormone.
In horses with PPID, the pituitary gland no longer works properly: it produces too much ACTH, causing the cortisol level in the blood to rise. This disrupts all kinds of body functions, from the sugar balance to the immune system.
⚠️ Why a raised ACTH does not always mean Cushing’s
There are various reasons why a horse can temporarily have a higher ACTH value without there being any question of PPID.
Seasonal influence
In autumn (especially August to October) the ACTH value rises in all horses, including healthy ones. This is a natural reaction to the shortening days. That is why, during that period, higher reference values are used for interpretation.
Stress - pain - illness
In the case of stress, pain or illness, the body naturally produces more cortisol to cope with it — that is a normal defence mechanism. The pituitary gland stimulates that cortisol production by producing extra ACTH. The result is that the ACTH value rises temporarily, even in a healthy horse.
Examples of situations that can cause this:
a horse that gets worked up during the blood draw,
transport, competition stress or a change of stable,
pain due to laminitis, colic or lameness,
or an underlying infection.
If the ACTH rise is the result of stress or pain, the value usually normalises on its own once the horse calms down or recovers.
That is why it is important, in case of doubt, to repeat the test in a calm, stable period and to assess the result together with the clinical picture.
Medication or feeding
Medication (e.g. corticosteroids, dopamine agonists, etc.) can influence the ACTH value, directly or indirectly. Always tell your vet which substances the horse has recently received, so that the blood test is interpreted correctly.
A raised ACTH value must therefore always be interpreted in context, together with the clinical signs and history.
🔍 Diagnosing PPID
The diagnosis is not made on the basis of a single blood test alone. The vet always looks at the overall picture.
Symptoms
coat changes
muscle loss
lethargy
fat deposits above the eyes or at the crest of the neck
unexplained laminitis.
Blood tests
ACTH value, possibly in combination with a TRH stimulation test to see how strongly the pituitary gland responds.
Repeat test(s): after a few weeks, certainly if the result is doubtful or after starting treatment
Only when both the symptoms and the blood values match can one say with certainty that a horse has PPID (Cushing’s).
💊🌿 Treatment
The standard treatment consists of herbal therapy and/orpergolide, a medicine that mimics the action of dopamine. It inhibits the overactive pituitary gland and helps to restore the hormone balance.
Important:
The dose is adjusted individually.
Always start gradually, to avoid side effects.
Regular monitoring of ACTH and the clinical picture is needed to assess the effect.
In addition, management remains crucial:
A tailored ration 🍃
Restricted grazing for horses with insulin resistance
Good hoof care and regular checks for laminitis
A calm environment and routine to reduce stress
In some horses that receive pergolide, we see a clear reduction in appetite or apathetic behaviour. In these horses it is important to check whether they really do have Cushing’s, whether the dosage is not too high and whether all the other pieces of the puzzle fit. It may possibly help to replace the pergolide with tailored herbal therapy.
🧩 The link with laminitis
PPID is one of the most important causes of laminitis in older horses. The raised cortisol level affects the sugar balance and makes tissues more susceptible to inflammation. That is why a horse with Cushing’s can develop laminitis without being too fat or eating a lot of sugar.
Early recognition and treatment of PPID is therefore essential in order to prevent hoof problems.
🌿 Living with Cushing’s
A horse with PPID can, with the right care, still live comfortably for many years. Many animals respond excellently to treatment and regain their energy, appetite and coat shine.
It is important to:
check the ACTH value annually,
adjust feeding and exercise,
and remain alert for signs of relapse or laminitis.
💬 In summary
A raised ACTH value is not automatically a diagnosis of Cushing’s, but an indication that further investigation is needed. By taking season, stress and symptoms into account, your vet can make a reliable diagnosis and start a targeted treatment.
